Confidential Family Survey
Confidential Family Survey
California utilizes a formula, called the Local Control Funding Formula (LCFF), to create funding targets based on student characteristics and one of the targets is based on free and/or reduced price meal status. In order to determine this status, we've created a confidential family survey for each household to complete. Aside from helping determine each school's state and local education funding, the income data provided by this survey will also help with consideration for family fee waivers for other school programs.
You may download the confidential survey below and submit the completed form to your school's main office, or you may pick up a copy to complete in your school's main office.

Paradise Unified School District uses the same Enrollment Form for all students attending any of our schools. To download a PDF version that you may print, fill out and take to the school of your attendance area click on the appropriate link below. If you are in need of an Inter-district Agreement Form you will find an electronic version below as well.
*There are additional required forms to be picked up from the school in order to register your student.
